In this game, players take on the role of a "Tooth Fae," a servant of the Tooth Fairy tasked with harvesting teeth from sleeping individuals. The gameplay is straightforward yet suspenseful. Using only a mouse, players must carefully extract teeth from patients' mouths without waking them. Success depends on managing three key meters: lucidity, pain, and fear. If any of these meters reach their peak, the patient will awaken, and the player will be caught.
To aid in their grim task, players have a variety of tools at their disposal. These include a needle for numbing, a hook to retract the gums, and pliers for extraction. There is also "pixie dust" to help keep the patient asleep. For more stubborn teeth, a drill is available, but it must be used with caution to avoid waking the patient.
The game features a rogue-lite element, where players choose from a selection of houses, each containing a randomly generated patient with unique traits. Some patients may be "tough," while others are "scaredy cats," affecting the difficulty of the extraction. The ultimate goal is to collect a variety of teeth to complete a set. However, if a player is caught, that house becomes unavailable for the remainder of the game. The game ends when there are no more houses to visit.
